When our organization went to file our RRF-1 for 2009 recently, we
discovered belatedly that we were not registered in the California
Registry of Charitable Trusts. I apologize for this oversight: I
thought we had registered when we first began receiving donations, but
I was mistaken -- I forgot to register us.

Therefore I am enclosing a CT-1 Initial Registration Form along with
our RRF-1, along with two separate \$25.00 checks, one for each form.
We understand that there may be a penalty for our late filing of the
Initial Registration Form, and I have already left a message at the
Californa Office of the Attorney General inquiring about this.

For reference, our California state corporation number is
{\b 2945514}, our Federal EIN number is {\b 261798415}, our phone
number is {\b (312) 772-2726}, and I can be reached at the email
given earlier. If you require further clarification for any of the
above, please do not hesitate to contact me.

\closing{Sincerely,}
